Privacy and Personal Information

Your personal information is protected by law, the Privacy Act 1988 (Cth) (Privacy Act) and the Privacy and Data Protection Act 2014 (PDP Act). We collect personal information that we believe is reasonably necessary to provide you with the services we offer. This includes information you provide to our staff verbally or otherwise and any information we may need to collect about you from third parties on your behalf (if applicable).

Personal information includes your name and contact details and sensitive information, for example about your health. This includes information about your NDIS plan and assessments, supporting documents and information relating to your health, safety, and wellbeing, such as medical records. By providing us with this personal information you consent to our use or disclosure of this information for purposes related to the services we provide. Unless permitted or required by law, we will not use this information for any other purpose without your consent.

In certain circumstances, Omega is bound by mandatory reporting responsibilities to the National Disability Insurance Agency, the NDIS Quality and Safeguards Commission, Department of Families Fairness and Housing, the Coroners Court of Victoria, Office of the Public Advocate and Victoria Police.

Access to Personal Records

When we hold your personal information, you have the right to request access to this information. If you want to request this information from Omega, you can call to ask or you can put your request to us in writing. If you would prefer, you can nominate a support person, advocate, or guardian to access these records on your behalf.

When we receive your request, we will make sure that we have the right consent. We will collect the information you have asked for and we will do our best to provide the records to you within 14 days.

You may ask us to update or correct any information in your personal record that you believe is incorrect.
